Live Well Kent is an integrated service that helps Kent residents to improve their mental and physical health and wellbeing, giving them the support, information and skills they need to build a healthy, independent life.
Live Well Kent is a free service for anyone over 17.
Our service model
Our aim is to connect people with their communities, ensuring they have access to the widest possible range of community support and services to meet their particular needs.
We believe in a ‘no wrong door’ approach: connecting people with appropriate services in a seamless way. We work together with other local charities and service providers to make this happen.
The programme is delivered on behalf of Kent County Council and the NHS by two charities, Shaw Trust and Porchlight, in the following areas:

|Thanet||Canterbury and Coastal|
|South Kent Coast|
